Replacement for lost or stolen license     $20

The applicant requesting a replacement license must complete a notarized affidavit form. An addition $5.00 is charged if the Barber Commission provides the notary service for the affidavit.

License renewals are due by January 31st of each calendar year. A late fee of $5.00 per month is charged beginning February 1. Licenses may be renewed beginning October 1 for the following calendar year.
